Scheduling appointments

There are a variety of scheduling options available, including same-day appointments, and we will make every effort to schedule your appointment at a time that is convenient for you.

As a courtesy to others who may require our services, we ask that you provide 24 hours notice when canceling an appointment.
 

What to bring to an appointment

  • Insurance cards
  • Photo ID
  • Co-payment


Co-pay’s are collected at the time of check-in, so patients need to be prepared to pay at that time. We take cash, check and all major credit cards.

All self pay patients will be expected to pay at the time of service with one of the above approved methods of payment.
 

If you are a new patient, please complete this New Patient Registration Form and bring it with you to your first visit.
 

For caregivers

Due to space availability in the patient exam rooms, we required a limit of one person accompanying the patient to this area of the office. If you need more then one person to accompany you, please let our receptionist know so we can make every effort to accommodate your needs.
